Bronchogenic carcinoma
CXR at gap of 2 years. Note on the recent CXR the right paracardiac region shows a new opacity, which appears to be continuous with the right descending pulmonary artery, but in fact it is a tumour in the right lower lobe.

Sacral metastasis
Ill defined sclerotic area on plain radiograph. The same region shows increased uptake on bone scan, and sclerotic deposit on coronal CT.
